UPPER, LOWER EYE LIFTING
Eyes age quicker than the rest of human face. A person may have cheeks and lips still young with a fine contour and volume, but droopy upper eyelids or eye bags make them seem older. It is true that after some disappointing experiences with cosmetic or aesthetic medicine treatments, patients finally contact a plastic surgeon. Mesotherapy, Botox, Pellevé, Plex R or Dermapen are all very interesting treatments, but, none tackles serious problem such as excess sagging skin, droopy eyelids or eye bags. To successfully resolve such issues Dr. Osuch often suggests upper eye lifting, lower blepharoplasty or both combined under the same anesthesia. It is important to know that eye lifting does not address small wrinkles around the eyes nor sagging eyebrows. For such issues Botox injections of eyebrow lifting are recommended.
Eye lifting is the most effective method of eye rejuvenation. It gives spectacular and long-lasting results. Various combinations such as upper eye lifting under local anesthesia, lower blepharoplasty under local anesthesia or sedation, upper and lower blepharoplasty under sedation are possible. Eye lifting might also be combined with other procedures such as eyebrow lifting, lip lift, lip implant or facelift.
Blepharoplasty will be of the greatest benefit to people who have:
- Excessive sagging skin on the eyelids
- Excess fat deposits (“eyebags”) underneath the eyes
- Drooping eyelids limiting the field of vision or hampering make-up application
- Eyes constantly tired and ‘closed’, and a worn look
Online consultationHow it looks like?
In case of upper eyelift, the surgeon makes an incision in the eyelid fold, cuts off excess skin, models the orbicularis oculi, and removes fat deposits if there is such need. The incision is closed with a fine surgical suture, approximately the thickness of a hair. The procedure lasts about 45 minutes and is most frequently performed under local anaesthesia.
Lower eyelid surgery entails an incision just below the eyelash line. Sagging skin, part of the muscle, and excess fat tissue (hernias) are removed or repositioned. In justified cases, lower blepharoplasty is combined with canthoplasty (correction of the side ligaments). Upper and lower eyelift might also be combined in one anaesthesia.
Before and after a lip lift
Patient underwent upper and lower blepharoplasty. Excessive fat tissue was repositioned from underneath the eyes. Excess sagging skin from the upper eyelid was cut off. As a result, the patient achieved spectacular rejuvenation. No additional surgeries were done.
Upper blepharoplasty patient. Excess, puffy skin on the upper eyelids was removed to get rid of a worn and sleepy look. As she had perfect eyebrows, no eyebrow lifting was performed.
This patient had lower blepharoplasty done to reposition fat deposit underneath her eyes. After getting rid of eye bags she looks completely different, with refreshed eyes which greatly contribute to her whole face younger appearance. Her upper eyelids were not touched; no surgery was performed in this area, but generally it is possible to combine both, typically under sedation.
Sometimes upper eye lifting is performed on one eyelid only like it was in case of this patient. She came to us as she experienced eye asymmetry and drooping of a right eyelid which hindered proper vision. The second photo was taken six months after the levator aponeurosis advancement. This is an example of an eye lifting done for medical reasons, for improving her life quality. Of course upper blepharoplasty also improved her appearance, but this patient was interested mainly in improving her eye function.
Upper eye lifting patient. The procedure was done under local anaesthesia. Excessive skin on her eyelids to was removed to “open” and refresh the eyes area. As she has perfect eyebrow line, no
brow lifting was necessary, but very often these two procedures are combined under one anaesthesia.
Upper blepharoplasty patient. Under local anaesthesia excess skin which fell over the upper eyelids was removed. Results: refreshed and rejuvenated appearance, eyes more open.
This patient had upper blepharoplasty. Thanks to removing excess sagging skin, her droopy eyelids which made her eyes sad-looking were eliminated. She told us that she feels 10 years younger.
This patient was treated with upper blepharoplasty under local anaesthesia. Especially on closed eyes you can see substantial excess skin on upper eyelids which made her look sad and sleepy. During surgery this tissue was removed. She got the result of more open eyes, preserving her own facial features.
This patient had upper eye lifting performed to address drooping eyelids. The second photo was taken just six days post-op, just a moment after stitches removal. That is why a scar is quite visible and still red. It will whiten with time and will be invisible with open eyes. Six days after upper blepharoplasty patient typically experiences bruising and edema; both subside within two weeks.
Recovery and the scar
Patients leave the clinic approximately one hour after the treatment (if it was performed under local anesthesia) or after 24 hours if the surgery was performed under sedation. They leave with a small dressing on the eyelids. That is why if you are coming for a blepharoplasty, remember about sunglasses. It will help in comfortable journey back home after your blepharoplasty procedure.
Bruising and edema appear a few hours after the procedure in the eye area. They typically subside within 1–2 weeks. Stitches are removed 5–7 days post-op. Full recovery time requires up to 10 days.
In case of an upper eyelift, the scar is located in the natural crease of the upper eyelid, and it is easy to be concealed with make-up. In case of lower blepharoplasty, the scar is located approximately 2 mm below the lower eyelash line. For stitching we use fine sutures, the thickness of a hair, which leave extremely thin scars. With time the scar becomes whiter, thinner and less visible.
To ensure best appearance of the scar:
- Do not expose it to the sun for the first 12 months post-op
- Apply sun blockers during sunbathing
- Apply silicone gel after the surgery
- Do not stretch the area where the scar is located for one month

1Who’s a good candidate for eye lifting?
People with the following issues: eye bags, fat deposits underneath the eyes, and those who want to refresh their eyes.
2Can dark circles under the eyes be treated during eyelift?
No. Eye lifting improves skin tightness, eye bags and treats excess skin. All types of discoloration may require other cosmetic or dermatological treatments.

Blood tests & anesthesia
1What anesthesia you use for blepharoplasty?
For upper eye lifting Dr. Osuch generally uses local one. For lower blepharoplasty – local anesthesia or sedation are used, dependent on an individual case. Sedation and general anesthesia is always connected with up to 24 hour’s stay in the clinic.
2What blood tests are needed for blepharoplasty?

Stitches & scar
1When stitches should be removed after blepharoplasty?
For blepharoplasty we use traditional, non-dissolvable stitches. You should have them removed 7 days post-op. Out-of-country patients typically arrange for stitches removal by a GP in their home country.
2How a scar looks like?
Upper blepharoplasty: the scar is located in the natural fold of the eyelid. It is easy to be covered with an eye shadow.
Lower blepharoplasty: the scar is located just below the line of the lower eyelashes (around 2 mm below). It is easy to be covered with a concealer. For eye lifting we use fine stitches to ensure the tiniest scar possible. With time the scar fades and get less visible, though it never disappears.
3I want eyelid surgery, but I'm afraid of a scar.
Like any surgery, eye lifting is associated with a scar. It may be more or less visible but it will never disappear completely. If you are afraid of scars, look for less invasive methods of eye rejuvenation. Perhaps the effect will not be as spectacular as after blepharoplasty, but the treatment will be scar-free.
4How the dressing after blepharoplasty looks like?
You are leaving the clinic with stitches and white plasters on your upper or lower or both eyelids, depended on what procedure was done. You can find out how exactly it looks like
here. To make your return home comfortable – remember about taking sunglasses with you.
5When can I sunbathe after blepharoplasty?
About a year post-op. Do not expose any fresh scars as they “catch” the sun very quickly and become brown. Discoloration is then difficult to be removed. After eyelift you need to protect the eye area by applying a cream with a high solar filter (SPF 50+).
After eye lifting
1How recovery looks like?
You are leaving the clinic the same day and can fly back home right away. At the end of the surgery, the doctor puts stitches on your eyelids. They should be removed 7 days post-op. Bruises/ swelling in the eye area can remain for about 10 days. After roughly 10 days post-op you can come back to your work but remember about taking frequent breaks to make your eyes comfortable.
2When can I start applying make-up?
After stitches removal, around 7 days post-op. To avoid any side effects please remember about applying make-up with clean hands and use fresh cosmetics.
3When can I come back to gym?
After 1 month post-op.
4Can I get back home by car after the procedure?
No, self-driving on the same day is not recommended after any surgery.
